3. Please check your 'Junk', 'Spam' or 'Promotions' folders if you have not receive the email containing the Consent and Intake forms.

If you still have not received the Consent and Intake forms, please call us and leave a message - we will call you back as soon as it is feasable.

*If you are a paying Privately or an International Client - you do not need a Mental Health Care Plan and referral letter.
